Transition Planning/Adult Services
Service Description
This service provides a focused, structured review of existing educational and transition-related records to help families better understand strengths, gaps, and areas that may warrant further clarification. Documents may include: • Current IEP • Transition plan and assessments • Neuropsychological evaluations • Progress reports • Agency correspondence Following review, families receive consultative feedback highlighting observations, questions to consider, and potential areas for further discussion within existing systems. Format: Virtual consultation Length: 60–75 minutes Includes: Pre-session document review (IEP, evaluation, or transition plan) Optional add-on: Written summary with recommended action steps. Consultation services are educational and preparatory in nature and do not include legal review, formal evaluation, representation, or attendance at school meetings.
