Service Description
Ages 14–22+ Preparing for adulthood involves more than transition goals in an IEP. Many families are surprised by how complex the shift from school-based services to eligibility-based adult systems can feel. This service provides structured, consultative guidance to help families understand commonly available Massachusetts programs and prepare application materials with clarity and organization. Areas of Focus: • Understanding the shift from entitlement to eligibility-based systems • Exploring programs such as DDS, SSI, MassHealth/CommonHealth, PCA, and vocational rehabilitation • Timeline planning for key milestones (ages 14–22+) • Customized documentation checklists to streamline preparation • Guided review of completed applications prior to submission • Community-based independence training Services are educational and consultative in nature. Final submission of applications and eligibility determinations remain the responsibility of the family and respective agencies.
